D.W. McMillan Home Health – a full-service home health agency that provides quality post-acute care and other home health services to patients in Brewton, Escambia County and surrounding counties – was recently recognized by D.W. McMillan Memorial Hospital for achieving a perfect score on its second quarter 2016 Centers for Medicare and Medicaid Services (CMS) quality metrics. The Agency’s perfect score was announced shortly after the completion the Agency’s recent quarterly internal quality audit.
Earlier in August, the agency held a congratulatory breakfast with Chris Griffin, D.W. McMillan Health System Administrator. In addition, the agency staff was honored a few days later by the hospital in a ceremony with hospital department heads. The staff was again congratulated by Mr. Griffin and presented with an honorary plaque commemorating their achievement.
“The real honor is being able to work every day with the healthcare professionals at D.W. McMillan Home Health to provide Brewton, Escambia County and the surrounding region with quality post-acute and other home health services,” said Neva Moore, the agency’s director. “Our patients place their trust in us and our level of service, and I am proud of our agency staff and their hard work in achieving such high marks.”
Working together, the home health agency and the hospital offer home-based solutions that are demonstrably effective in decreasing the number of avoidable hospital readmission days. Working with an experienced and proven home health provider helps patients seamlessly transition from the hospital to home. Home health care is convenient, cost-efficient and supports faster recovery and patient independence.
