Brewton Middle School fifth students and parents should have received a form to fill out regarding immunizations with reports cards recently.
HN Immunizations will partner with BMS to provide three vaccines for children at school for no cost.
The T-DAP is required by law for all students going into sixth grade.
The other vaccines will be offered and are optional and are Meningococcal and HPV or Gardasil.
School officials urge parents to at least take advantage of the T-DAP.
Upcoming sixth graders will not be allowed to start sixth grade without the vaccine.
Deadline to return forms is April 12. If the child has insurance, that information need to be provided.
